Had an intersting one today, fitted a contatum RCBO some time ago to a socket ring to comply with RCD regs, today said RCBO went up in smoke, apparntly it was buzzing with a bit of smoke then the buzzing stopped with lots of smoked traced fault to kettle but thats a bit concenring, could have set on fire!! Asked customer if they had pressed trip button and obviously hadn't so guess it must have got stuck. Anyone else had this problem before??

 
Don't use contactum, but fitted Controlgear RCBO (Denamns Baisc) 2 weeks ago, returned to continue job last week to find it was 'off' but still passing electric so basically failed in a dangerous state (though might have been something to do with plumber). Shan't be using again.

 
Recent industry research on in service reliabilty of RCDs resulted in a finding of approx 4% failure rate....in the majority of cases of failure No recent testing had been carried out on the installations and the occupiers confirmed they had never performed the recommended Quartely manual trip test..........

Incidentally some RCBOs manufacturers recommend a monthly test on their devices......in practice these routine tests are never carried out and in essence the guidence is really only a get out clause for the manufacturer when a situation such as you describe ,in your post, occurs Blue Fox .
