We should like to provide a link to a questionnaire, regarding risk assessment, and any general H&S paperwork you may do in the course of your work.

Although the questionnaire is anonymous, there are 20 prizes being offered, if you wish to submit your details.

Before we provide the link, the management of TEF Solutions would like to make the following clear:

• The questionnaire is not connected to http://www.talk.electricanforum.co.uk (TEF Solutions Limited) in any way, and we have no control of the actions of a third party, or their web site / hosts etc.
• http://www.talk.electricanforum.co.uk (TEF Solutions Limited) has been assured that any personal details, email addresses etc. will be used for the notification of the prize winners only, and will be removed from all databases immediately afterwards; although we cannot independently verify that.

Those members choosing to enter, and leaving their details, will be entered into a draw, to win one of 20 £5 "Amazon" vouchers. The draw is being run completely by the third party.

The results of the questionnaire are being used to determine the feasibility of a company to help deal with the paperwork; and the forum will have access to the results, as well as being credited on the published results.
