100A TP switch fuse, been wired the wrong way, feed from meter goes through fuse, then to switch, then to SWA. so if you switch off, the fuseholder is still live.

Code 2?

also

SWA leaves switch fuse and goes to DB approx 15m away. there is no banjo's on the SWA going to earth bar, just glanded directly to metal case. havent taken a Zdb yet, but some Zs's are well within limits. codeable?

 
I would say that was a code 1 requires immediate attention.

The fact that parts are still live constitute a failure of the primary function.

The swa may be ok if the earthing through the metal can be verified.

If not then it would be requiring improvement.

 
Andy,I think you're right about a code 2 for the switch fuse, but you don't have to use a banjo when glanding to a MC enclosure so long as you clean the paint off!!
Thanks. Zdb and Zs's are very good, so i dont think there is any additional resistance between gland and metalwork.
