It could if section A, termination of the 4core swa, or earth attachment from the banjo within the dist-board or earth attachment within the 25amp isolator. Wrong size cable from the 25amp isolator to the 16amp 5pin socket [line and cpc sizes].
The AM2 assessor should have given you feedback after the first attempt if he worth his position.
