I did some pat test forms and put them into the downloads section a little while back, its on an excel spreadsheet that has most of the certs all on one workbook.

Avoid Seaward, expensive, unreliable and the support from them is very poor from experience :|

Not sure what kit you have got but if EasyPAT from Tysoft is as good as Easycert, its probably good value for money and they do support their products.
